What is the TM Flow study?
The TM Flow study (also known as the TM Flow System) is a cutting-edge diagnostic platform that combines multiple technologies into a single, non-invasive, outpatient test.
It assesses multiple aspects of vascular and ANS function, including:
- Sudomotor (sweat-gland) function analysis, to evaluate small-fiber neuropathy and ANS involvement
- Photoplethysmography (PPG) to estimate skin blood flow and micro-circulatory perfusion via infrared light sensors at the fingertip
- Ankle-Brachial Index (ABI) with pneumoplethysmography to screen for peripheral arterial disease (PAD) and atherosclerosis in the lower limbs
- Autonomic reflex testing (deep breathing, postural changes) to measure the interplay of the sympathetic and parasympathetic systems in real time

Who can benefit from a TM Flow study?

- Numbness, tingling, and burning sensations in the hands and/or feet (typical of small-fiber neuropathy)
- Autonomic symptoms such as lightheadedness, dizziness upon standing (orthostatic intolerance), resting tachycardia, syncope, or POTS-type presentations
- Known diabetes mellitus or pre-diabetes/insulin resistance, where early autonomic and vascular changes are common and often underdiagnosed
- Signs or risk of peripheral arterial disease (PAD), such as leg claudication, cold feet, or known cardiovascular disease risk factors
- Chronic pain in the feet, trunk, or extremities, gastrointestinal issues such as abnormal sweating, bladder dysfunction, or sexual dysfunction — all of which may be related to autonomic or small-fiber nerve involvement
Additionally, the TM Flow study may be appropriate when standard lab tests and routine clinical evaluations do not fully explain a patient’s symptoms, and further insight into autonomic or circulatory health is needed.
